I confirm that I am over 18 years of age and that I am not currently under the influence of alcohol or recreational drugs. I understand that a tattoo is a permanent mark on my body, and I give my full consent for Sophie to perform the tattooing process today.

I trust Sophie’s professional discretion regarding the final design and the technical process required to achieve the best result for my skin. I also agree to my tattoo being photographed and used for media or promotional purposes to share the story of our work.

I understand that once I leave the sanctuary of the studio, the health and longevity of the tattoo are my responsibility. I agree to follow the aftercare instructions provided to ensure the mark heals as intended. Finally, I consent to my personal information being kept on file for a minimum of two years, as required for professional record-keeping.
